About Us
Located adjacent to the Rogue Valley Medical Center, Hearthstone is committed to offering quality care to patients in an environment that meets all the needs of our residents and the families we serve. We provide for each resident’s total care — including individual physical, social, emotional and spiritual needs — and encourage and welcome input, involvement and support from our residents and their families. We’re proud of our rehabilitation program, which encourages independence and activity and helps each resident acquire new skills and relearn old ones. Our dementia program offers a holistic approach that addresses the needs of residents confronting Alzheimer’s Disease and related dementia.

Blake Epp, Administrator

 

Blake was previously the administrator of Lincoln City Rehabilitation Center. His relationship with Pinnacle started when he took a nursing assistant class offered by one of Pinnacle’s facilities in Roseburg. In 2007, he started work as a certified nursing assistant (CNA) for Umpqua Valley Nursing and Rehabilitation Center. Soon he was promoted to social services assistant and eventually became director of social services. Later, after completing an administrator-in-training (AIT) program at Umpqua Valley, Blake accepted the assistant administrator position at South Hills Rehabilitation Center in Eugene.

 

A graduate of Oregon State University with a degree in biology, Blake says the best part of his job is the interaction with people. He especially enjoys getting to know the senior citizens he meets through his work. He says he is honored to serve the generation that helped form the world in which we live.

Shirley Collins, RN, Director of Nursing Services
Shirley has been at Hearthstone Nursing and Rehabilitation Center since 2002, when she took on the role of resident care manager. She accepted the position as director of nurses in 2005. The majority of her career has been spent in long-term care in California, where she worked as an MDS coordinator, an assistant director of nurses and a director of nurses.

Shirley enjoys spending time with her three grown children, grandchildren and other family members living in the Rogue Valley. Shirley says her favorite part about working at Hearthstone is the daily interaction with residents.

Rhonda Brodie, PTA, Rehab Program Manager
Born and raised in Roseburg, Ore., Rhonda came to Pinnacle after leading a successful military career. Following high school graduation, she joined the United States Air Force, and in 1981, while still in the service, she earned her Certified Occupational Therapy Assistant (COTA) degree. Rhonda went on to serve in the USAF for 10 years, working as a COTA.

Following discharge from the military, she then worked as a COTA in varied settings in the community. In 2007 she returned to Oregon and joined the team at Hearthstone Nursing and Rehabilitation Center, becoming the rehabilitation program manager in 2009. Outside of work, she enjoys spending time with her three children.
